Upon arrival to our office, you'll be asked to fill out forms regarding your current medical condition and previous medical history. This is a necessary part of the evaluation and examination process. Also, it enables us to complete insurance claims on your behalf.
Health insurance is filed as a courtesy by our staff. We ask that you please provide us with current and accurate information regarding your insurance coverage.

All co-payments are expected at the time of service. Any outstanding balance that remains following payment from your insurance company is due within 30 days. Balances over 90 days accrue interest at a rate of 12% APR. Please contact our billing department with any questions during business hours. We accept cash, checks, MasterCard and VISA.
